How to Scan the Appendix – Part 1: Principles

In this first part of the series, we focus on the essential principles for successful appendix ultrasound:

🔹 Graded Compression Technique
Gradually increase pressure to displace bowel gas and bring the appendix closer to the probe, improving visibility.

🔹 Proper Transducer Selection
Start with a low MHz curved or microconvex probe to survey deep structures, then switch to a high MHz linear probe for fine detail and confirmation.

🔹 Two-Probe Approach
Combine low-frequency and high-frequency transducers to overcome challenges in both obese and thin patients.

🔹 Maximal Tenderness Probe Technique
Place the probe exactly over the patient’s point of maximal tenderness—this often leads you directly to the appendix.

🔹 Finding the Ileocecal Valve as a Landmark
The ileocecal valve is a reliable landmark; once identified, carefully trace the cecum to locate the appendix.

👉 Mastering these principles is the foundation of confident appendix scanning.
