Online Heart Failure Resources for Doctors and Patients

The following are some online resources for diagnosis and treatment of heart failure that are useful to both patients and physicians.

The VA has a heart failure toolkit for providers at https://www.queri.research.va.gov/chf/products/hf_toolkit/ which contains many links to outstanding clinically applicable resources for both inpatient and outpatient heart failure care.

The Heart Failure Society of America has a heart failure website at https://www.heartfailureguideline.org/ and they have their complete 2010 guidelines on line at https://www.heartfailureguideline.org/guideline_sections/41

Two heart failure questionnaires that can be useful in following heart failure are the Kansas City Cardiomyopathy Heart Failure Questionnaire and the Minnesota Living With Heart Failure Questionnaire.

The Kansas City Cardiomyopathy Questionnaire is available as a pdf at https://www.queri.research.va.gov/chf/products/hf_toolkit/KansasCity-CardiomyopathyQuestionnaire_Spertus.pdf

The Minnesota Living With Heart Failure Questionnaire is available at https://www.queri.research.va.gov/chf/products/hf_toolkit/Minnesota-HF-Questionnaire_Rector.pdf

If you are a patient or a doctor, review both of the instruments and pick the one that seems to work best for you. By printing and completing a new form every month (pick one or the other) you will have an addition useful resource for tracking your progress.
