Support the channel on Patreon: patreon.com/drsamsimaginglibrary Coronary CT Angiography (CCTA) Normal (CAD-RADS 0) Vs CAD-RADS 3 Cases | Moderate Atherosclerosis CAD-RADS = Coronary Artery Disease – Reporting and Data System It’s a standardized reporting system for coronary CT angiography (CCTA) that grades the severity of coronary artery disease (CAD) and gives management suggestions. CAD-RADS 0 No coronary artery disease, absence of plaque 0% stenosis No further evaluation required CAD-RADS 3 Moderate coronary artery stenosis 50–69% luminal narrowing in at least one coronary artery. Indicates obstructive CAD is possible, but not yet severe. Management: Non-invasive functional testing (e.g., stress perfusion imaging, CT-FFR, stress echo, nuclear MPI). If symptoms persist or high-risk features are present → consider invasive coronary angiography. Medical therapy (antianginal medications, statins, risk factor control) is generally indicated. Lifestyle modification remains critical. Plaque Burden: P1: Mild plaque burden. Plaque limited to 1–2 coronary segments. Usually focal, early atherosclerosis P2: Moderate plaque burden. Plaque involving 3–4 segments. More diffuse than P1, but not extensive. P3: Severe plaque burden. Plaque involving 5–6 segments. Considered high-risk extent. P4: Extensive plaque burden. Plaque in ≥7 coronary segments. Very high atherosclerotic burden → higher event risk.
